New Patient Registration

We are registering new patients. We are offering face to face appointments if requested, but can also do telephone and video consultations if preferred.

You may register with us if you are living within our Practice boundary. You will need to complete a registration form at the surgery or complete one online and email it to us as below.

It would be helpful if you bring your medical cards for this. If these have been mislaid we will still be able to help you. When you register with the practice you will be asked to complete a health questionnaire.

As no one is automatically registered with a surgery, it is important that both you and all members of your household register at the same time. We recognise the important role of carers in looking after dependent relatives; please make it known if you are caring for someone.

Who Will Be Your Doctor?

When you register we will confirm with you which of our surgeries is the most appropriate for you to attend. Your registration will be with the Practice. You may choose to see any of the Doctors at your registered surgery. For continuity of care in ongoing problems, we would encourage you to consult the same Doctor as far as possible.

Changes in Service Provision

Please note that the information provided on this website is current at the time of going to print, however, changes to the Service Provision will occur from time to time which will render the booklet to be temporarily out of date. To compensate, it will always be useful to observe the posters and leaflets on display in the Waiting Rooms. If you have a particular enquiry please ask a member of staff to help you.

Consulting with Young People

If you are under 16 years of age and need to consult with a health professional in this Practice we ask you to attend with a parent, your legal guardian or a responsible adult nominated by you or your parent/guardian. However, if you wish to see the Doctor or Nurse on your own this can be arranged. Sometimes, the Doctor or Nurse may ask permission from you and the adult who is with you if he or she may speak to you privately. All consultations are strictly confidential but if you are under 16 years and consult alone you will be asked if your parent/guardian is aware of the consultation. A parent or guardian will not be told of your consultation without your informed consent unless the clinician believes that to not tell a parent, guardian or third party of the consultation would be harmful to you or place you in danger. We are duty bound at all times to act in your best interests and recognise your rights and values in all consultations.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
